Please fill out the appropriate BODY RELEASE form (if applicable when body is in an Autopsy Morgue) Either “Central Texas Autopsy” or “Travis County Medical Examiner”

Please fill out the ‘Family Authorization to Cremate’ paperwork.

Please sign at the BOTTOM of the “Authorization to Embalm” document to ‘decline authorization to embalm’ if you choose a DIRECT CREMATION. This allows for a direct cremation WITHOUT VIEWING. There is NO VIEWING if a DIRECT CREMATION is chosen.  Other choices are available, contact us for more information.

If you wish to pay by credit card, you can fill out the “Credit Card Authorization” and email or fax it to us or you can simply pay by calling in your information.

Use the ‘Obituary Template” to write a UNIQUE obituary for your loved one.  Attach a photo to use with your obituary.

Please fill out the forms, sign them where required, and then use your SCANNER on your PRINTER and save each individual page and send me an email to: bob@eedsfuneralhome.com with your new scans as attachments…OR…you can send everything (except the photo) via FAX to 512-398-9523.  Please do NOT use your phone camera to take pictures and email them to me, because phone cameras usually will not arrive in a readable format.  Scan and email, or fax the documents, please.
